Access your GP online services
With Evergreen Life, your patients can access NHS-assured patient services securely using the only app connected to all three major GP systems in England. This means that the app is connected to 99% of GP practices in England. Online access to GP services through the app allows patients to easily book GP appointments, order repeat prescriptions with home delivery, and access their GP medical record in the palm of their hand.
Discover your Wellness Score
Once your patient has linked to your practice, they can use our app to better understand their own wellness, starting with questionnaires that we call Wellness Checks. Completing these questionnaires will reveal a 'Wellness Score,' that indicates whether you're doing all you can to stay well.
The questions in the Wellness Checks are referenced and reviewed by our team of clinical wellness experts, combining knowledge from medical professionals, alternative medicine approaches, and personal trainers. Over time, we’re able to suggest changes to improve a person's wellness with practical insights and personalised recommendations. These suggestions can help patients to practise self-care strategies and manage their health more independently.
Build an accurate Personal Health Record
With our app patients can integrate their medical record with their own personal health records, so they always have the most up-to-date record of their health in their hands, empowering your patients to always be in control. A Personal Health Record (PHR) is an online record owned by the individual. It allows patients to add and organise their lifelong health information that may not be held in the GP record. Patients can track health conditions, set medication reminders, and monitor health and fitness measurements. Users can also add in any documents and letters that they may have received from healthcare providers.

Having access to the right information at the right time is key when it comes to providing care for your patients. In the Evergreen Life app, your patients can give trusted access of their health record to those who need it - whether it's a family member, friend or healthcare provider. Users can share as much or as little information as they would like - the level of trusted access is determined by the person and they can change it at any time. Each recorded piece of information within the GP medical record or personal health record has the option to make it 'private,' if they don't wish for that information specifically to be shared.
DNA testing: Unlock your health secrets
Evergreen Life also offers at home DNA testing kits which can be added to the account via the app. With a DNA kit, you can understand more about your health and wellbeing by revealing your genetic insights. The four sections in the DNA results include: Diet, Metabolism, Fitness and Skincare. With this information, users can make informed lifestyle choices to take control of their health, fitness and wellbeing.
