Letter to GP practices asking for GP linking to be enabled
If you've found that your practice does not enable patient facing services, this letter template can help you request online record access from your GP practice and provide guidance on how they can set this up.
You may have tried to link to your GP previously with Evergreen Life and found that it did not work. This could've been because your GP practice had not yet enabled patient linking. That may have changed since you last tried. However, if you have since requested new linkage codes which still have not worked, this letter template can help you formally request online record access from your GP practice and provide guidance on how they can set this up.
Simply copy the following template into an email or document to edit:
To [GP PRACTICE NAME],
As a patient at your practice, I wanted to register for online services, wanting to book appointments, order repeat prescriptions and access my GP records online. I chose Evergreen Life as my provider of this service.
When I tried to register, I found that your practice does not allow me to link to get any of these functions. That makes my healthcare less safe and less convenient. It also means that the practice is not fulfilling its contract with the NHS.
