How to view your GP record

Be more connected to your GP-held medical record by downloading it into your Evergreen Life account.

To do this, you'll need to create an Evergreen Life account on the app or website and link to your GP system. If you haven't done this already, you can GP link remotely using NHS login - a single username and password that allows you to access your GP services in minutes and without leaving home. Alternatively, you can link using a registration letter that your GP practice provides.

  1. Open your app and log in. If you haven't got the app yet, you can download it here.
  2. Tap the 'GP' section. If you haven't yet linked, you can link remotely using NHS login from this screen.
  3. Tap the blue 'Go to GP Services'
  4. Tap 'View Medical Record'

So, what do these different sections mean?

Summary

The summary section of your GP record contains an overview of your medical history, including any clinical problems/conditions, allergies you've been diagnosed with and medications prescribed. The medications here are grouped by acute (short-term) and repeat prescriptions.

Consultations

You'll be able to view any notes made during your consultations at your GP practice. This can be invaluable information when you have multiple appointments for the same condition at various different places. For example, you may have to visit the hospital where they have no record of the information held by your GP.

Medications

Tap on 'Medications' to see a list of acute and repeat prescription medications that you have been prescribed. This is a complete list of all medications prescribed by your NHS GP.

Immunisations

Here you'll find a list of every vaccination or immunisation you have had with your GP practices.

Results

If you have a blood test or another medical test at your GP practice, you may be able to see the results here.

Letters

In this section, your GP practice may give you access to view any documents or letter correspondence here.

  1. Log in to the website or create an account.
  2. After logging in, go to the green section called 'My Medical Record' and click confirm your identity. If you haven't yet linked, you can do this remotely using NHS login here.
  3. Tap 'Summary' to see an overview of your GP record
  4. Next to 'Summary,' you'll see a list of tabs - these are the parts of your GP-held medical record that your practice has shared with you
