Changing GP practice

If you have recently moved to a new GP practice, you may have noticed that your GP online services are no longer working. To regain access to these services, you'll need to relink your Evergreen Life account to your new practice. You can see how to do this below.

You'll first need to unlink your account from your old practice:

  1. Log into your account
  2. Tap 'GP NHS', then 'Go to GP Services'
  3. Tap 'My GP Account'
  4. Scroll to the bottom of the screen and tap 'Unlink GP Account'

Once you have successfully unlinked your account, you will be able to link to your new practice by requesting that they provide you with an online registration document with the required codes and entering these details as you did originally.

** If you find that you cannot unlink via the app, please attempt via the website before contacting us **

  1. Go to Settings / My GP
  2. Click 'Unlink' at the bottom of the screen and confirm that you wish to remove access.

Once you have successfully unlinked your account, you will be able to link to your new practice by requesting that they provide you with an online registration document with the required codes and entering these details as you did originally.
