How to link my GP record with my Evergreen Life account with registration letter
I have a letter from my GP to register for online services. How do I use this to link to my Evergreen Life account?
You can connect your Evergreen Life to your GP medical record, keeping all of your health and medical information in one place. Once connected, you can use GP online services within Evergreen Life such as booking appointments, ordering prescriptions and viewing your medical record.
When you go to your GP practice, you'll need to request a registration letter for online services from your Practice (photo ID will most likely be required). All NHS GPs are required to provide this letter when requested. If the practice staff are unsure or do not provide you with this letter, please get in touch and we can help you get connected.

How to link to your GP record on the Evergreen Life app:
- Open the app and log in to your Evergreen Life account. Don’t have the app? Download it now.
- Tap the 'GP' section and tap on the blue 'Link to my GP' button.
- If you have a letter from your GP, tap ‘Yes, I have them’ to continue. You will require information that is within this letter; if you have not yet received it, please contact your practice and ask that they provide you with your online registration document.
- Enter your practice code into the text box on the next screen. This will be a 6 digit code in the format A12345. Please note that the first letter is always upper case.
- Your practice details should appear on screen. Tap on the address practice.
- On the prompt ‘Is this your surgery?’ tap 'YES, let’s go' if correct.
- On the next screen is GP System Registration Terms of Use, read and tick to confirm if you agree to the terms, and tap ‘Next’.
- A Security Check window appears, tap next then enter your account password when prompted.
- Enter the Account ID and Linkage Key where prompted. Both are case sensitive.
- Enter your surname and date of birth then tap ‘Register’
- Once complete, a window with ‘Registration with GP system was successful’ will appear. Tap OK.
That’s it, you should now be linked. Have a look around the ‘GP System’ section of the app and see which services are available.

How to link to your GP record on the Evergreen Life website:
- Go to the Evergreen Life website and log in to your account. If you don’t have an Evergreen Life account, you can create one here.
- Click on ‘Settings’ at the top of the screen.
- Click on ‘My GP’ from the settings drop-down menu.
- Enter the site code (ODS code) as it appears on your registration letter. If the letter does not contain a site code, enter the Practice name or postcode instead.
- Click on ‘Find Practice’ then select your Practice from the list of sites that appears.
- Click on ‘I have a letter or email’.
- Enter your account ID and linkage key/passphrase as it appears on the registration letter. All fields are case sensitive.
- Enter your date of birth.
- Click ‘Link Account’.
That’s it, you should now be linked. Have a look around your account home page and see which services are available.
